      PLEASE NOTE:  Registration is per family so only register for one attendee, unless you need two rooms per the hotel/FAM policy where you can only have 4 people in a room.  ONLY immediate family members living in the same household are permitted to attend for the weekend.  There is a Saturday only option that is open to extended family members this year, although that does not include a hotel room. Per FAMOhio's policy, all families will have their registration confirmed by their local HTC and/or Chapter.  By registering you are giving us permission to confirm this information.  If you have questions about this policy please send a note to prior to registering. Once your registration is confirmed and your fee has been paid, you will be sent documents via email that must be filled out and returned in order to finalize your registration. ~~~~~~~~~~~~~~~~~ Each year, nearly 400 adults and children in the bleeding disorder community from across Ohio come together to share an incredible weekend of education, activities, networking and fun! FAMOHIO, Inc. starts its planning each calendar year, meeting quarterly or as needed, to develop the best weekend possible. Meals, lodging, and activities for all registered family members will be covered by your registration fee for up to 4 immediate family members living in the same household. Any additional hotel charges for example, room service, phone calls, video rentals, etc. are the responsibility of the attendees.  In exchange for this deeply discounted registration fee, attendance at Education Sessions is MANDATORY for all attendees.  Individuals who fail to attend the Education Sessions may not be invited back the following year. The registration deadline is Friday, July 12, 2019. Any registration postmarked or received online after that date will be placed on a waiting list and may not be accepted to attend FAMOHIO based on space limitations.  If you are placed on a waiting list you will be notified if we have a cancellation. Hotel policy limits each room to 4 people. If you register and your family exceeds 4 people, another room must be purchased for an additional fee. Sorry, no exceptions! Also, per hotel policy:  A Credit or Debit Card will be required when you check in at the hotel for any incidentals. PLEASE NOTE: Use of a debit/check card as payment for your incidentals will immediately place a hold on funds in your checking account. The amount of the authorization/hold requested will be approximately $50 per day. Your financial institution may continue to hold these funds for 3-5 business days after you check-out. Neither FAMOHIO nor the hotel can accept responsibility for overdraft charges resulting from holds placed on debit/credit card accounts. You will only need to submit ONE registration per family, unless you need a second room. We will offer childcare programs at the hotel for infants (3 months-36 months), children (37 months-6 years & potty trained), youth (7-9), youth (10-12), teens (13-17), and young adults (18-22). To guarantee your child has a space in the activities throughout the weekend, you must register your child by July 12th. Registration for children, youth, and teens will NOT be accepted after July 12th. For those parents with infants, please note there will be no hotel cribs to use during the weekend. BACK this year:  Saturday ONLY option.  The cost is $25 per family, up to 4 people, and includes all meals, Educational Sessions and evening activity at the hotel on Saturday night.  Does NOT include a Hotel room. Once registered and your information has been verified by your HTC and/or local Chapter you will be sent forms for registration and childcare that MUST be mailed/emailed by July 19th.
